Contact Lens Re-Fill Request

Welcome! To order a contact lens re-fill, complete the form below.

We offer two options:
  • Free shipping to your home (within Canada) for minimum 2 box/$100 purchase. $10 flat rate shipping for smaller orders.
  • Pick up in clinic. We will order the lenses and e-mail you when they arrive (payment at pick-up*). No shipping charge.
*Some exceptions apply for custom made-to-order lenses which require pre-payment

Important details:

  • Valid, unexpired prescription required. We will contact you to arrange an exam if you submit a request and your prescription is expired.
  • For ship-to-home orders, contact lenses will not ship until e-invoice is paid.
  • We will attempt to bill your insurance on file (if applicable) and only invoice the remaining balance.
  • If you are not a patient, we will contact you for further information to request a copy of your prescription.
  • Our best price guarantee and returns & exchange policy applies to all purchases.
  • We cannot ship to PO boxes, international addresses or addresses not served by couriers.
